How do I request Mental Health Records?
Texas and Arizona law requires that requests for mental health records be made in writing.
To obtain your records, or your child’s records, please follow the steps below:
Submit a written request
Choose one of the following options:

Via Patient Portal Complete our HIPAA-compliant Authorization form and submit it through the patient portal of our electronic health records system Sessions Health.

The specific records you are requesting

The name, address, and/or email address of the intended recipient

Instructions for Attorneys and Document Companies
A subpoena alone is not sufficient to compel disclosure of confidential counseling or billing records, or other Protected Health Information (PHI), under HIPAA (45 C.F.R. Chapter 164).
For mental health or medical records requested or subpoenaed in litigation (including testimony), HIPAA permits disclosure only under the following circumstances:

In response to a valid court or administrative tribunal order, or
-
When the individual (or parent/legal guardian):

Is a party to the proceeding
-
Has notice that the PHI has been requested
-
Does not object to the disclosure
An Authorization from the individual or parent satisfies this requirement.

Important Note
If the client or parent has indicated that they do not consent to release of records:

A “Statement of Assurance” will not be accepted

A valid Authorization or Court Order will be required

Hi there, thank you for taking the time to be curious about my work! I possess an innate desire to understand each individual and couple’s unique perspectives and circumstances. This allows me to create a safe non-judgemental space for clients in the therapy room. As a counselor, I am here to be a collaborative and interactive part of your healing journey by using your feedback along with my knowledge and skills to aid you in a deeper understanding of yourself and/or your relationship. We will dive into the roots of your presenting concerns by digging into your past including your childhood while simultaneously focusing on the current manifestations of these issues as they show up in your adult relationships. Together, we will find coping strategies, and begin to bring awareness to your habitual, and reactive behaviors- all while processing life events that shaped you. You will also receive homework and tools to use outside of our sessions to aid in deepening our work in a sustainable way. I work from a trauma-informed, mindfulness-based, and holistic lens where we move with intention. I enjoy working with clients on fostering the connection between the mind and body. It is powerful and where I lean into my yoga teacher training.

My father is a Sri Lankan immigrant, and my mother is a white American. My childhood was a blend of two cultures from people who were trying to figure out how to navigate the differences themselves after marrying young. Growing up, I did not know many people who identified the way I did ,which was confusing. I was raised in Arizona, where it felt like the question of my heritage came up frequently. I felt that I did not fit in fully in either culture, and that felt very isolating. My experiences being bi-racial and bi-cultural led me to become passionate about working with minority groups.
